Care services
Personal Care
This includes assistance with showering, bathing, dressing, and help to get up from bed or returning to bed. We'll be able to visit you at a time that works best for you to help make your mornings more manageable.
Medication Supervision
We can visit to ensure your correct medication is taken at the prescribed times and the prescribed dosage. These can be 30 minute welfare checks depending on the location and flow of the roster.
Registered Nurse care
From time to time, people require a registered nurse for professional input and assurance. One of our friendly nurses will set up your support with PHC and visit regularly to keep your care plans up to date and meeting your changing needs. Our RN’s oversee all of our care and our team are able to call them for support at any time of their shift.
24-hour care
Some people wish to remain in their own homes, and we are privileged to provide a luxury 24 hour service. This service is comprehensive. You will have your own team and can feel at peace knowin your chosen plan for this vulnerable time will be carried out the way you would like it. Your life, your choice.
Sleepovers
Some people require night care due to there being a high fall risk or night care assistance. We can supply overnight sleepovers care, and awake nights. Different rates are available depending on the care level needed.
Dementia Care
Due to cognitive decline, living at home can be a challenge. We can visit for an allocated time to allow safety and companionship. We can also come to offer respite for family members who are the primary carers. Remember you need rest too.
Convalescence/Rehabilitation Care
Often after surgery, hospitalisation or similar, support is needed to help you recover. We can also help with any exercises or rehabilitation programs given by physiotherapists. We are ACC accredited and can work in conjunction with their service. We are proud to carry MI, RTI and the ACC nurse contract for our area.
Palliative Care
We at PHC value this service highly and treat this care with the sensitivity needed to help families through this precious time. We often work in conjunction with Otago Community Hospice. We offer visits during the day or overnight, allowing relatives/full-time carers to take a break knowing that their loved one is still being cared for during a very difficult time.
Welfare Check
This service is helpful when only 15 mins are needed. We can check in to ensure all is well and that medication is being taken. This is only offered if the client lives nearby other work we cover, and the time works with other cares in the community.
Registered Nurse Phone Support
This service is helpful when you or your family want to talk to a registered nurse on the phone. This is not an on call service as a time will be arranged that suits you both. Rates are charged per 15 mins.
General Services
Companionship
Spending time with people is important for well-being. We can provide a social setting with coffee in a cafe or a gentle listening ear over a cuppa at home. If there are hobbies or events you would like company to attend, this service is wonderful for enjoying all aspects of social interaction.
Shopping
Often its more difficult to get out to the shops and we can either accompany you or carry out the shopping on your behalf.
Hospital & doctor visits
We can accompany you to your appointment or drive you to a medical, physio, or dental visit. This can be purely for support/another ear or for transport purposes.
Visits to other towns and cities
On occasion, some people may want to visit family/friends in another city or even another country, and we can accompany you on these trips.
Outings
No matter whether its summer or winter an outing is enjoyable and we often hear clients say, “it’s just lovely to get out of the house.” We can provide a picnic or an ice-cream along the way. Winter is a great time to take a flask and sit in the warmth of the car down by the lake.
Referrals
From time to time people require increased services and assistance from outside agencies, at which time we can assist with negotiating the way through the health system. We can assist with referrals to Physiotherapist, Occupational therapists, Needs Assessors/Social Workers.
Equipment hire
Often we have wheelchairs, shower stools and toilet raisers for hire. These will be delivered and fitted as required. We have a safety sensor system for clients with a high falls risk. This provides safety while staying at home.
We are team players
Where possible Personal Home Care will work in partnership with your Te Whatu Ora provider to support a good care team for people in the community. PHC works in collaboration with all of our networking services. We have good working relationships with the Wanaka District Nurses, Otago Hospice, medical centres and care homes. We are all on the same team and willing to lend a hand where needed.
Additional Services
Meal preparation
Meals are often one of the activities that become more difficult as people get older. We can prepare and cook meals either daily or cook various meals for the freezer, to be used later in the week. If the meals are in the freezer we are then able to visit and heat them up and serve them to the you in the comfort of their own home.
House Cleaning
We have a trusted cleaner who supports our team. She comes with her own cleaning gear, and her rate is slightly cheaper. She will focus only on cleaning. This includes dusting, vacuuming, and washing the hard floors. The kitchen and bathrooms will all be spotlessly cleaned. We also change beds, wash the linen, remake the bed/s and do any required ironing. Ovens, microwaves, cupboards, drawers and fridges are also on her list. She is a home care angel.
Gardening
Depending on availability at the time. Gardening is offered when possible.
Window cleaning
We can arrange for your windows to be cleaned. We have two trusted window cleaners who live in our local community.
